Instructions & Suggestions for printing and use of CAPF 73.
  1. Download one copy of the CAP Form 73 in Word format
  2. Fill in all the data that does not change.  Wing/Region: SER-FL, Charter Number: FL-001, Vehicle Identification No. (VIN), Year of Vehicle, Vehicle Make, Vehicle Model, Field ID No.: (Bumper Number)
  3. Fill in Month & Year. i.e. 2012/January (Use this format, it will make it easer to change the month)
  4. Print on copy for January, front (current) page only.
  5. Change the month and print February. Continue to do steps 4 & 5 until you have make up forms for each month of the year.
  6. You may then print page two on the back, head to foot. This makes it easer to write on the form in the log book.
  7. Three hole punch the pages in place them in Tab 3, in front of the previous years completed forms.
  8. Suggestion that you place the current CAPF 73 in the front of the log book so it will be easy to locate and work with.
  9. At the start of each month conduct your initial safety/maintenance checks, indicated that you have accomplished each item with a check mark or comment. Record any problems or concerns VEHICLE OPERATOR DISCREPANCY REPORT on the back of the sheet. When completed, sign the bottom of the ITEMS TO BE CHECKED DAILY block at the bottom. If your signature is not recognizable, print in your name and rank. Sign the form with the date the initial inspection was conducted.
  10. On the previous months CAPF 73 enter the end of month mileage in the upper right corner. CAP VEHICLE INSPECTION GUIDE AND USAGE DATA. "END OF MONTH ODOMETER READING."
  11. Submit your CAPF 73 report to National & FLWG No Later Than 1800 hrs. on the 5th of each month.
